Finasteride lowers DHT, the hormone that shrinks follicles in pattern hair loss. Oral finasteride 1 mg is FDA-approved for men; topical and low-dose forms are compounded and used off-label.
Minoxidil promotes hair growth and lengthens the growing phase. Topical minoxidil 2% and 5% are FDA-approved for men and women; the 7% and dissolvable-strip forms here are compounded.
Tretinoin helps minoxidil penetrate the skin. Fluocinolone helps manage inflammation or irritation. Ketoconazole helps manage flaking and dandruff-related concerns.

It's more common than most people are told. In clinical trials, hair loss was reported by about 3% of adults taking Wegovy® (versus 1% on placebo) and 4–5% of adults taking Zepbound® (versus 1%) — and it was more often reported by women. It's usually not the medication itself. Rapid weight loss puts the body under stress and can push a large number of hairs into their shedding phase at the same time, a pattern called telogen effluvium. Shedding typically starts two to four months into weight loss and, for most people, settles as weight stabilizes. For some, though, the shedding reveals thinning that was already starting — pattern hair loss that would have shown up eventually. That's where prescription treatment can help, and it doesn't mean stopping your GLP-1. Yes. Women of any age can be prescribed a finasteride-free minoxidil spray. Postmenopausal women may also be offered finasteride-containing options, including oral finasteride, at their provider's discretion. Women who are pregnant, breastfeeding, or planning a pregnancy are not eligible for treatment.
Hair loss was reported in clinical trials of Wegovy® (about 3% vs 1% on placebo) and Zepbound® (4–5% vs 1%), more often in women. It's generally linked to rapid weight loss rather than the drug itself, usually starts a few months in, and often improves as weight stabilizes. If it doesn't — or if it's revealing thinning that was already starting — a provider can evaluate you for treatment without stopping your GLP-1.
In clinical studies, a small percentage of men (under 4%) reported sexual side effects such as decreased libido or erectile dysfunction, most of which resolved. Less common effects include breast tenderness, mood changes, and depression; the FDA has also issued warnings about mood-related effects, including with topical finasteride. Finasteride can cause birth defects and must not be handled by women who are or may become pregnant. Your provider will review your history before prescribing.
Topical finasteride delivers medication to the scalp and lowers, but doesn't eliminate, the amount that reaches the rest of the body. The FDA has noted reports of the same side effects seen with oral finasteride in people using compounded topical products. Neither route is side-effect-free; your provider can help you weigh them.
