How it works

Tirzepatide is a dual GIP and GLP-1 receptor agonist. It slows stomach emptying, reduces appetite and food noise, improves insulin release after meals and changes how the body handles stored fat. In the SURMOUNT-1 trial, adults on the 15 mg dose lost an average of about 21% of their body weight over 72 weeks with lifestyle support, and in SURMOUNT-OSA it reduced breathing interruptions during sleep by more than half.

Dosing & Titration

Zepbound® starts at 2.5 mg once weekly for four weeks, then increases in 2.5 mg steps every four weeks as tolerated: 5, 7.5, 10, 12.5 and up to 15 mg. Maintenance doses are 5, 10 or 15 mg. Each pen contains one dose; a monthly plan is four pens at the strength your provider prescribes, and every strength is the same price.

Inject under the skin of the abdomen, thigh or upper arm on the same day each week. Pens are kept refrigerated and can sit at room temperature for up to 21 days if needed.

Who it's for

Zepbound® is prescribed to adults with a BMI of 30 or higher, or 27 or higher with at least one weight-related condition, and to adults with obesity and moderate-to-severe obstructive sleep apnea. It's the right choice for people who prefer or require the FDA-approved product, who are continuing a Zepbound® prescription without insurance coverage, or who want pre-filled pens instead of drawing from a vial.

It is not for anyone who is pregnant, breastfeeding or planning a pregnancy, for people with type 1 diabetes, or for anyone with a personal or family history of medullary thyroid carcinoma or MEN 2.

What to expect, month by month

Month 1 (2.5 mg). The starting dose is for tolerability; appetite usually begins to settle and mild nausea, if any, fades within days of each injection.

Months 2–3 (5–7.5 mg). Dose increases bring a strong, steady effect on hunger. Most people see consistent weekly weight loss and fewer cravings.

Months 4–6 (10–15 mg). You reach a maintenance dose matched to your response. Changes in waist size, energy and blood pressure are typically clear by now.

Beyond 6 months. Weight loss continues toward a plateau; the trial ran 72 weeks. Your provider will plan long-term maintenance with you.

What is Zepbound® FDA-approved for?

Zepbound® (tirzepatide) is FDA-approved for chronic weight management in adults with obesity, or with overweight and at least one weight-related condition, as an addition to a reduced-calorie diet and increased physical activity. It is also approved for moderate-to-severe obstructive sleep apnea in adults with obesity. It is the same molecule as Mounjaro®, which is approved for type 2 diabetes.

How is Zepbound® dosed?

Treatment starts at 2.5 mg once weekly for four weeks, then increases in 2.5 mg steps every four weeks as tolerated: 5, 7.5, 10, 12.5 and up to 15 mg. Maintenance doses are 5, 10 or 15 mg. Each plan is one month of single-dose pens at the strength your provider prescribes; the price is the same at every strength.

How is Zepbound® different from compounded tirzepatide?

Zepbound® is the FDA-approved product made by Eli Lilly, supplied in pre-filled single-dose pens with fixed strengths. Compounded tirzepatide contains the same active ingredient but is prepared by a compounding pharmacy as a multi-dose vial, allowing intermediate doses and a lower price, and it is not FDA-approved. GobyMeds is not affiliated with Eli Lilly.

Why does the brand-name medication cost more than compounded?

Brand-name pens are manufactured by the original pharmaceutical company, FDA-approved and sold at the manufacturer's list price; GobyMeds adds no markup beyond the pharmacy's cost and the included provider visit. Compounded versions are prepared by licensed pharmacies for individual prescriptions and cost considerably less, but they are not FDA-approved. Your provider can discuss which option fits your situation.

Can I use insurance or a manufacturer savings card?

GobyMeds does not bill insurance, and manufacturer savings cards generally can't be applied to a cash-pay telehealth order. If you have coverage for this medication, your own physician and in-network pharmacy are usually the lower-cost route; we're a good fit when coverage is denied or unavailable and you want a fast, transparent cash price. HSA and FSA cards are accepted.

Is the brand-name medication in stock?

Supply of brand-name GLP-1 pens has fluctuated over the past few years. We show current availability on this page and confirm it again before your order is sent to the pharmacy. If a strength is temporarily unavailable, your provider may suggest an alternative strength, a short bridge on compounded medication, or waiting for restock, and you'll never be charged for medication that can't be shipped.

How is brand-name medication shipped?

Pens ship refrigerated from a licensed U.S. pharmacy in insulated cold-chain packaging with a signature typically required on delivery. Most orders arrive within two to five business days of approval. Refrigerate pens on arrival; an unopened pen can be kept at room temperature for up to 21 days (Zepbound®) or 28 days (Wegovy®) if needed.

What is your cancellation and refund policy?

If a GobyMeds provider decides a prescription isn't appropriate for you, you receive a 100% refund of the medication price (lab fees, where they apply, are not refunded). Once your order has been sent to our medical partners or to the pharmacy for fulfillment we will do our best to accommodate changes or cancellations, but we can't guarantee them, and in some cases an adjustment or cancellation carries a $50 medical consultation fee.

Because of regulations covering prescription medication, we are unable to accept returned medication for a refund.

Do I need insurance, and can I use my HSA or FSA?

No insurance is needed and GobyMeds does not bill insurance. The price you see is the price you pay, with provider consultation and shipping included and no membership fee.

HSA and FSA cards are accepted at checkout, and we can provide a letter of medical necessity if your plan administrator asks for one.

How does the online visit work?

You answer a short health questionnaire and upload a photo ID. A licensed U.S. provider reviews your history, confirms the treatment is appropriate, and writes the prescription. There is no video call to schedule unless your provider or your state requires one, and follow-up messaging with your care team is included with every plan.

Important safety information

Side effects and precautions

Common side effects are nausea, diarrhea, vomiting, constipation, stomach pain, indigestion, injection-site reactions, fatigue, allergic reactions, belching, hair loss and heartburn, most often during dose increases. Serious risks include pancreatitis, gallbladder problems, kidney injury from dehydration, low blood sugar with other diabetes medication, diabetic retinopathy complications, severe allergic reactions and depression or suicidal thoughts.

Zepbound® carries a boxed warning for thyroid C-cell tumors observed in rats. Seek medical care for severe abdominal pain, trouble swallowing, hoarseness or a neck lump. Full prescribing information is available from Eli Lilly; report side effects to the FDA at 1-800-FDA-1088.

Who should not use it

Do not use Zepbound® if you or a family member has had medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2, if you have had a serious allergic reaction to tirzepatide, or if you are pregnant, breastfeeding or planning a pregnancy. Do not combine with another GLP-1 medication. Tell your provider about pancreatitis, gallbladder, kidney or eye disease, depression, and all medications you take; oral contraceptives may be less effective for four weeks after starting and after each dose increase.
