How to Administer an Intramuscular Injection

For Testosterone Replacement Therapy (Injectable Testosterone Cypionate)

Intramuscular injections are used to deliver medication deep into muscles for effective absorption. These injections are used for testosterone therapy and are the "gold standard" because they allow for slow, sustained absorption of the medication to help maintain testosterone levels over time. Areas for injection are deltoids, vastus lateralis (outer thigh), and upper outer quadrant of the gluteal muscle.

Important Information about Needles and Syringes

Due to the viscosity of the testosterone solution, a thicker gauge needle is used to draw the medication from the vial, and a thinner gauge needle is used to inject into the muscle.

Your testosterone order includes syringes with a luer lock, which allows for a screw-on-off connection to securely attach the needle.

Phase 1: Preparation is Key

Before you start, find a well-lit, clean space.

• Wash and Dry: Thoroughly wash your hands and dry them with a clean towel.

• Gather Supplies: You will need your medication, syringes, needles, alcohol prep pads, and a sharps container. It’s also helpful to have clean gauze or a bandage nearby.

• Inspect Your Meds: Double-check that the name on the vial is yours, it’s the correct medication, and the dosage matches your prescription. Check the "BUD" (Beyond Use Date) on the label. Testosterone should be stored at room temperature to make it easier to draw in to the syringe and avoid "crystallization."

Phase 2: Prepping the Site and the Vial

• Choose Your Site: For intramuscular injections, the best spots are the deltoids, vastus lateralis (outer thigh), and upper outer quadrant of the gluteal muscle.

• Pro Tip: Make sure to rotate your injection site with every dose to keep your skin healthy.

• Sanitize: Clean the chosen site with an alcohol pad for 3 to 5 seconds and let it air dry.

• Prep the Vial: Pop the cap off the vial if it's new. Use a fresh alcohol pad to wipe the rubber stopper on top.

Phase 3: Drawing the Medication

• Attach the needle: If needed, attach the thicker-gauge needle (21G) by screwing it onto the luer lock on the syringe.

• Add Air: Remove the cap from the needle. Pull the plunger down to your prescribed dosage level to fill the syringe with air.

• The "Vacuum" Trick: Insert the needle into the vial at a 90-degree angle and push the air in. This prevents a vacuum from forming, making it much easier to draw the liquid out.

• Extract: Turn the vial upside down and pull the plunger back to your exact dose.

• Clear the Bubbles: Hold the syringe upright. If you see air bubbles, tap the side of the syringe until they rise to the top, then gently push the plunger to expel them. It’s okay if a tiny bit of liquid comes out.

Phase 4: The Injection

• Switch needles: Unscrew the needle from the syringe and screw the thinner-gauge needle (25G) onto the luer lock on the syringe.

• Inject: Insert the needle swiftly and smoothly at a 90-degree angle into the desired injection site. Slowly push the plunger all the way down.

• Finish & Dispose: Pull the needle out, apply pressure, and massage the area to disperse the medication. Apply gauze or a bandage if needed. Immediately place your used syringe and needles in a sharps container. Never throw needles in the regular trash.
